    PAYMENT & REGISTRATION PROCESS

    To guarantee entry into class, please fill out the registration form and hit 'Submit'. Then print, sign, and mail in the Service Agreement (below), along with your payment prior to the start of classes.
    If we do not receive your paperwork and payment before the start of classes, we can not guarantee you a spot. Each class has a limited number of spots available. Registration is first come, first served.

    CASH
    Cash payments will be collected in the first week of class. In order to reserve your spot, we will need your registration form and the service agreement before the start of classes, so please submit your registration form now, and mail in the service agreement ahead of time. First come, first served: Please note that non-paid spots will be held as long as there is availability. Should we receive payments for all available spots, we will not be able to provide a spot for students who have not yet made payment.

I, _________________________________________________________, understand that Z&E Canine Manners, LLC will make every effort to provide my dog with a safe and clean class environment. I understand that Z&E Canine Manners, LLC will check the vaccination records of all participants as proof that the dogs in the class are current on required vaccinations. Regardless, I understand and acknowledge that exposure to other dogs may still carry the potential risk for the transfer of illnesses. I accept all responsibility and costs incurred, and will not hold liable now or in the future the Trainer, Nildan M Atkay, Z&E Canine Manners, LLC, their employees, affiliates, and families should my dog get sick and/or require medical attention.
 
I also understand that Trainers and employees and affiliates of Z&E Canine Manners, LLC will take every precaution necessary to ensure the safety of my dog during group class interactions. Should my dog act in a way that is not appropriate or safe for a group class setting, I understand and accept without argument that I may be asked to take my dog out of the group class temporarily or permanently. Should there be an altercation, or should my dog attack another dog or person, or get attacked by another dog, I accept all responsibility, medical and legal costs incurred, and will not hold liable now or in the future the Trainer, Nildan M Atkay, Z&E Canine Manners, LLC, their employees, affiliates, and families.
